Car in reverse - but goes forwards!

Doesn't happen often, but I put my car in reverse sometimes, and it goes forwards.

 

It's in position for reverse, but it goes like in first (which can be quite dangerous)

 

Any ideas?

Gear linkage needs setting up (there was a post on here earlier that had pics on how to do that)

 

And/or the circlip under the gaitor is loose.

 

My 2 guesses anyhoo.

 

When the linkage was out of line on mine, I could sometimes get it into 1st instead of reverse, but not done it the other way round.
