312 upgrade

Can someone remind me what's needed for the 312 conversion please?

Carriers, calipers (if you don't already have 288mm brakes), Hose with banjo fitment (late mk3 golf which uses 288mm will do fine), discs and a spacer to centralise the caliper to the disc, from memory it's a 6mm but can't remember.
